First Visit Guide
Step 1: Confirm Your Appointment
Our team will call you to confirm your appointment as the date approaches. A verbal confirmation is required to hold your appointment. If you need to cancel or reschedule, we kindly ask for at least 2 business days’ notice. Cancellations without sufficient notice may incur a $55 fee.
Step 2: Gather your Dental Records and Bring the Essentials
- Valid ID
- Dental Insurance
- List of medications
If you’ve seen another dentist previously, having access to your dental records can be helpful. These records can give Dr. Hendricks and our team a more complete understanding of your dental history. You can request that your prior dentist send your records directly to our office, and we're also here to assist if needed. If you plan to use dental insurance for your visit, please provide us with your insurance details prior to your appointment so we can verify your benefits and eligibility.
Tip: Speed up the registration process by completing your new patient forms online before your appointment or arriving 10-15 minutes early to finalize paperwork and get settled in.
Step 3: Note Any Tooth Pain or Sensitivity
If you’ve been experiencing any tooth pain or sensitivity, make a note of it to discuss with Dr. Hendricks during your appointment. While we can identify issues during your exam, patient feedback is invaluable in personalizing your care.
Step 4: Keep Brushing and Flossing
Arriving with clean teeth helps us get a clear, accurate view of your dental health. We recommend brushing after each meal and flossing daily. These habits, along with regular dental visits, are key to maintaining a healthy mouth and a radiant smile!
Patient Registration Forms
Once your initial appointment is scheduled, you have two options for completing your new patient registration forms:
- Interactive Forms: Complete the forms online before your appointment to streamline the registration process.
- Downloadable Forms: Download, print, and fill out your forms at home. Remember to bring these completed forms with you to your appointment.
Note: For your privacy, please do not email these forms to our office. Email is not HIPAA-compliant, and we prioritize the security of your Protected Health Information (PHI).
Financing Information
